  • Mobile outpatient physical therapy is exactly like traditional outpatient PT — the same skilled, doctoral-level care, the same evidence-based treatments — except we bring it to your home. Instead of you traveling to a clinic, your therapist comes to you. Sessions are conducted wherever is most useful for your recovery.

  • In-home PT is ideal for older adults and anyone recovering from surgery, illness, or injury who finds it difficult to consistently attend a traditional clinic. It is especially valuable for patients who cannot drive during recovery, who are near-homebound, who manage a neurological condition, or who want more personalized, private care.

  • Traditional home health PT requires patients to be certified as homebound and is provided through a licensed home health agency. Eleet Health is a private-pay outpatient practice — you do not need to meet homebound criteria or have an agency referral. You simply need a physician's referral or prescription for PT.

  • Eleet Health is a private-pay practice. We do not bill insurance directly. However, we provide detailed superbills for patients with PPO or out-of-network benefits, which you can submit to your insurer for potential reimbursement. Contact your insurer to understand your out-of-network PT benefits before your first visit.

  • Initial Evaluation: $200. Follow-up visits: $175 per session. No hidden fees or surprise bills.

  • Your initial evaluation takes approximately 30-60 minutes. Your therapist will review your medical history, assess your strength, balance, mobility, and gait, evaluate your home environment for safety, discuss your goals, and develop a personalized care plan. You may begin treatment during the first session.

  • We serve Livermore, Pleasanton, Dublin, San Ramon, Danville, Alamo, Walnut Creek, Castro Valley, Hayward, San Leandro, San Lorenzo, Union City, Fremont, Newark, Sunol, and surrounding East Bay communities.

  • Most patients are seen 2–3 times per week, and the average plan of care runs 4–8 weeks. Your therapist will give you an estimate after your initial evaluation and reassess regularly as you progress.

  • Have a copy of your physician's referral if you have one. Wear comfortable, loose-fitting clothing. Clear a small open space in your living room — roughly 6 feet by 6 feet. We bring everything else.

  • A referral is not always legally required but is strongly recommended to coordinate care with your medical team and support any insurance reimbursement.

  • A superbill is a detailed receipt containing diagnosis codes, procedure codes, and your therapist's credentials that you submit to your insurer for reimbursement. We provide one at no extra charge for every visit.

  • Cash-based PT means we are not restricted by insurance rules on visit limits, treatment types, or session length. Our only focus is your recovery. When you factor in copays and deductibles, the cost is often comparable to a traditional clinic.

  • Our primary service is in-person, in-home PT. For certain follow-up consultations or exercise instruction, virtual visits may be available. Contact us to discuss your situation.
